Notification & Approval for research Related to Living Modified Organisms (LMOs)/Genetically Modified Organisms (GMOs)

  1. IBC Assessment of Project Proposal Involving Modern Biotechnology Activities
    • Form: Annex 2
    • Usage: This form must be used when the Institutional Biosafety Committee (IBC) evaluates contained use activities or field experiments before submitting the aplpication/notification form to Department of Biosafety.
  2. Incident Reporting (Non-Injury)
    • Form: Annex 3
    • Usage: This form must be used when there is an incident that does not involve an injury.
  3. Occupational Disease / Exposure Ibvestigation Form (Injury Involved)
    • Form: Annex 4
    • Usage: This form must be used when there is an incident involving an injury.
  4. Project Extension and Termination
    • Form:
    • Usage:
      • Annex 5A: Used by the Main Researcher to apply for time extension approval from the IBC. This form must be submitted at least one month before the end of the approved time period for the project.
      • Annex 5B: Used to inform the termination of the project to the IBC.

    • Forms:

Application for Approval for Field Experiments Involving Modified Living Organisms (LMOs)

  1. Approval for Release Activities of LMO (Research and Development Purposes in All Field Experiments) or Importation of LMO
    • Forms
      • Form A (for Higher Plants)
      • Form B (for organisms other than Higher Plants)
      • Annex 2 (IBC Assessment of Project Proposal Involving Modern Biotechnology Activities)

  1. AIMST Biorisk Assessment Form(s)
    • Usage: Replace with the latest version. Researchers should fill out this form to specify that the biological material they will be handling does not, in any way, require biosafety approval.
  2. Exemption Form
  3. Material Transfer Agreement (MTA) Form
  4. Chain of Custody Form
    • Form: Chain of Custody Form
    • Usage: Ensures the integrity of the biological materials being transferred or used in the research.
